Once the initial application has been enumerated and a National Provider Identifier (NPI) has been assigned, there are two options for changing/updating the information on an NPI record.

Making Changes Online

Online updates can be made by visiting and following the steps outlined below:

NOTE: Changes to Date of Birth, Social Security Number, Reactivation of an NPI, and Deactivation of an NPI cannot be completed online and require a Paper Application/Update Form.

  • On the Home Page of the NPPES website, enter your I&A User ID and password
  • Select the “Pencil” ICON to Edit the desired NPI application
  • Access the page containing the information to be revised by clicking on the ‘Next button at the bottom of each page or by selecting the correct page from the left-hand navigation bar.
  • On that particular page please enter new details.
  • After submitting all the details please submit it.
