What is an NPI number?
Covered health care providers, as well as all health plans and clearinghouses, must use NPIs in HIPAA-compliant administrative and financial transactions. The NPI is a 10-position numeric identifier.
The numbers do not contain any additional information about healthcare providers, such as their state of residence or their medical specialization. All applications and claims forms submitted under Medicare and Medicaid include an NPI number.
It is primarily used by healthcare providers to identify themselves in financial transactions, as well as by care plans to identify and coordinate with other providers.
Will a provider’s NPI ever change?
In most cases, a provider’s NPI cannot be changed. Unfortunately, if a health care provider (such as a physician) dies, his or her NPI is deactivated. Also if a physician ceases his services then also the NPI will get deactivated. The deactivated NPI will never be distributed to another health care provider. When a provider relocates from one state to another, the NPI number remains the same.

How do I change my NPI information?
Once the initial application has been enumerated and a National Provider Identifier (NPI) has been assigned, there are two options for changing/updating the information on an NPI record.
Making Changes Online
Online updates can be made by visiting https://nppes.cms.hhs.gov and following the steps outlined below:
NOTE: Changes to Date of Birth, Social Security Number, Reactivation of an NPI, and Deactivation of an NPI cannot be completed online and require a Paper Application/Update Form.
- On the Home Page of the NPPES website, enter your I&A User ID and password
- Select the “Pencil” ICON to Edit the desired NPI application
- Access the page containing the information to be revised by clicking on the ‘Next button at the bottom of each page or by selecting the correct page from the left-hand navigation bar.
- On that particular page please enter new details.
- After submitting all the details please submit it.
Making Changes with a Paper Application/Update Form
Updates can be constructed by sending a Paper Application/Update Form, which can be found at https://nppes.cms.hhs.gov or by emailing the NPI Enumerator and requesting one via mail.
